Overview
The Form Options section allows you to choose which information you would like to collect from members when they complete a membership application.
You can add or remove fields depending on the information your organisation needs to capture.
Every Membership Widget must include First Name and Last Name fields. The remaining fields are optional and can be selected based on your requirements.
This article explains the Form Options section of the Membership Widget.
Open Form Options
- Go to Admin > Widget Configuration.
- Find the Membership Widget you want to edit.
- Select Edit next to the widget name.
Please note: If you are creating a new widget, select Create Membership Widget. We recommend starting with the Form Setup article in the Membership Widget article series for guidance on setting up a new widget.
- Select Form Options from the configuration options on the left.
Contact Information
The Contact Information section allows you to choose which personal details you would like to collect from the member.

| Displays a text box for the member's home phone number. The information will be added to the contact as Home Phone. | |
| Displays a text box for the member's mobile phone number. The information will be added to the contact as Mobile Phone. | |
| Displays a text box for the member's email address. If an acknowledgement is being sent, this field will be required. | |
| Displays a calendar for the member to select their date of birth. The information will be added to the contact's Birth Date field. | |
| Displays a text box and an option for the member to sign up on behalf of an organisation. The organisation name will be added to the contact's Organisation field. If selected, the organisation will also be used as the contact's Account Name. |
Making a Contact Field Mandatory
When you enable a contact information field, an additional Mandatory checkbox will appear. Select this checkbox to require the donor to provide the information before they can continue.
Mandatory fields are identified by a red asterisk (*) next to the field label on the donation form.
Address Information
There are two ways to collect address information on the Membership Widget.
You can either use the Show Address option or use individual drop-down fields for the different parts of the address.
Option 1: Show Address
The Show Address option displays the address fields together and provides the member with the option to search for their address.
Depending on your settings, the address search can use Google or NZ Post.
Please Note: Do not use Show Address together with the individual address drop-down options below.
This option is useful when you want to make entering an address quick and simple for the member.
Option 2: Individual Address Drop-downs
Instead of using the Show Address option, you can use individual drop-down fields for the different parts of the address.
This option can be useful when you want members to select from predefined address options rather than entering their address using the address search.

Making an Address Field Mandatory
When you enable an address field, an additional Mandatory checkbox will appear. Select this checkbox to require the donor to provide the information before they can continue.
Mandatory fields are identified by a red asterisk (*) next to the field label on the donation form. The donor must complete the required information before proceeding.
Additional Information
The Additional Information options allow you to collect extra information from members or make it easier to match a membership application to an existing contact.

Displays the donation widget across three separate pages:
Donors use the Next and Back buttons to move between the pages. This can be useful when you want to simplify the appearance of the form by breaking the information into smaller sections. |

Changes the payment method selection from a drop-down menu to individual radio buttons, allowing the member to select their preferred payment option directly. |

Displays the GoDaddy verified site seal to the left of the Submit button. |

|
Displays a text box at the top of the form where an existing contact can enter their VegaID. This helps ensure the membership application is matched to the correct contact. |

|
Displays a checkbox allowing the member to purchase a membership as a gift for someone else. When selected, a contact form will appear where the purchaser can enter the recipient's details.

Set Mandatory Fields
Contact Information and Address Information fields can be made mandatory.
When a field is enabled, an additional option will appear allowing you to make that field mandatory.
When a field is set as mandatory:
- A red asterisk (*) will appear next to the field label.
- The member must complete the field before they can proceed or submit the membership form.
For example, you can make Email, Mobile Phone, Address 1 or Postcode mandatory if this information is required for your membership process.
Tip: Only make fields mandatory when the information is genuinely required. Keeping the form as simple as possible can make the membership application process easier for members.
