Donation Widget- Form Options

The widget configuration can be accessed from Admin > Widget Configuration:

Select the widget type and Edit to the right of the widget name you would like to edit:

Select Form Options from the configuration options on the left-hand side.

This section will allow you to add or remove certain fields, depending on the information you would like to capture.

Every donation widget must have the First Name and Last Name fields, the rest is up to you.

Form Option	Explanation
	This will display a drop-down with all title attributes
	This will display a text box and will be added to the contact as Home Phone
	This will display a text box and will be added to the contact as Mobile Phone
	This will display a text box and will be added to the contact as Email. If sending an acknowledgement this will be required
	This will display a calendar select and will be added to the Birth Date field on the contact
	This will display a text box and the option to donate on behalf of. This will be added to the Organisation field and if ticked the Organisation will be used as the contact Account Name

Form Option	Explanation
	This will display all address fields with the option to search for an address using Google or NZ Post, depending on your settings This should not be used as well as the drop-down options below
	This will display a drop-down of all attributes under a type called Suburbs
	This will display a drop-down of all attributes under a type called Cities
	This will display a drop-down of all attributes under a type called Regions
	This will display a drop-down of all Countries

Form Option	Explanation
	This will display a tick box and will process the donation as Payment type- In-Memoriam. When ticked a text area for details will display and this will be recorded on the donation particulars and as a note on the contact, Note Type- Donation Note
	This will display a text area for the donor to write a donation comment and this detail will be recorded as a note on the contact, Note Type- Donation Note
	This will display the widget in three individual pages, Donation information, Personal Information and Payment method, navigated with Next and Back buttons: This is a great functionality to simplify the look of your donation form
	This works when the Show Address option is selected. This will only display the search bar with a tick box to add address manually: This is a great functionality to reduce the fields on your donation widget and make the form simpler for your donors

Form Option	Explanation
	This will display the donation frequencies that have been added to the attribute type Frequencies. These are ordered on the widget as per the attribute sequencing
	The above option needs to be turned on first. This will display the frequency options as buttons instead of a drop-down
