Overview
The Form Options section lets you choose which fields and donation options are displayed on your Donation Widget.
You can customise the form to collect the information you need from donors while keeping the donation process as simple as possible.
Every Donation Widget must include First Name and Last Name. All other fields are optional.
Open the Donation Widget
- Go to Admin > Widget Configuration.
- Find the donation widget you want to edit.
- Select Edit next to the widget name.
Please note: If you are creating a new widget, select Create Donation Widget. We recommend starting with the Form Setup article in the Donation Widget article series for guidance on setting up a new widget.
- Select Form Options from the menu on the left.
Contact Information
Use the contact information options to choose which details you want to collect from donors.

Making a Contact Field Mandatory
When you enable a contact information field, an additional Mandatory checkbox will appear. Select this checkbox to require the donor to provide the information before they can continue.
Mandatory fields are identified by a red asterisk (*) next to the field label on the donation form.
Address Information
There are two ways to collect address information on the Donation Widget.
Option 1: Show Address
Enable Show Address to display the standard address fields on the donation form.
Depending on your organisation's settings, donors can search for their address using Google or NZ Post. The available address information is then populated into the relevant fields.
Use this option when:
You want donors to be able to search for their address rather than selecting each part of their address individually.
Option 2: Address Drop-downs
Instead of using Show Address, you can collect address information using individual drop-down fields.
Use this option when:
You want donors to select their suburb, city, region, or country from predefined lists rather than using an address search.

Making an Address Field Mandatory
When you enable an address field, an additional Mandatory checkbox will appear. Select this checkbox to require the donor to provide the information before they can continue.
Mandatory fields are identified by a red asterisk (*) next to the field label on the donation form. The donor must complete the required information before proceeding.
Additional Donation Information
These options allow you to collect additional information about the donor's donation.

Displays a checkbox allowing the donor to make the donation In-Memoriam. When selected:

Displays a text area where the donor can enter a comment about their donation. The comment is saved as a note against the contact with the Note Type – Donation Note. |

Displays the donation widget across three separate pages:
Donors use the Next and Back buttons to move between the pages. This can be useful when you want to simplify the appearance of the form by breaking the information into smaller sections. |

This option works when Show Address is enabled. Instead of displaying all address fields, the widget initially displays an address search bar with a checkbox allowing the donor to enter their address manually. This can reduce the number of fields displayed on the form and make the donation process simpler for donors. |
Donation Frequency

Displays the donation frequencies configured under the Frequencies attribute type. The frequencies are displayed on the widget in the same order as their attribute sequencing in Vega. |

This option must be enabled together with Show Frequency. It displays the available donation frequencies as buttons instead of a drop-down list. |

Displays an option above the donation amount buttons allowing the donor to select that they want to make a pledge rather than process a payment immediately. When the donor selects this option:
When Allow Pledge is enabled, additional options are available under Acknowledgements to select the email and attachment documents used for pledge acknowledgements. |

Displays the GoDaddy verified site seal beneath the Submit button on the donation widget. |

Where is the pledge option? It's listed on my widget settings but doesn't do anything when I enable it and any discussion of it is omitted from this page and elsewhere in the knowledge base.
